{"id":4,"date":"2016-06-01T18:07:13","date_gmt":"2016-06-01T18:07:13","guid":{"rendered":"https:\/\/www.vdh.virginia.gov\/care-connection-for-children\/?page_id=4"},"modified":"2026-03-26T20:41:00","modified_gmt":"2026-03-26T20:41:00","slug":"care-connection-for-children","status":"publish","type":"page","link":"https:\/\/www.vdh.virginia.gov\/care-connection-for-children\/","title":{"rendered":"Care Connection for Children"},"content":{"rendered":"<div id=\"pl-4\"  class=\"panel-layout\" ><div id=\"pg-4-0\"  class=\"panel-grid panel-no-style\" ><div id=\"pgc-4-0-0\"  class=\"panel-grid-cell\" ><div id=\"panel-4-0-0-0\" class=\"so-panel widget widget_sow-editor panel-first-child panel-last-child\" data-index=\"0\" ><div\n\t\t\t\n\t\t\tclass=\"so-widget-sow-editor so-widget-sow-editor-base\"\n\t\t\t\n\t\t>\n<div class=\"siteorigin-widget-tinymce textwidget\">\n\t<div id=\"LiveWell-banner\"><\/div>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"size-full wp-image-61 Picture_Right hidden-xs alignright\" src=\"https:\/\/www.vdh.virginia.gov\/content\/uploads\/sites\/61\/2016\/12\/home.jpg\" alt=\"Care Connection for Children\" width=\"250\" height=\"167\" \/><\/p>\n<p>Care Connection for Children is a statewide network of centers of excellence for children and youth with special health care needs (CYSHCN) that provide leadership in the enhancement of specialty medical services; care coordination; medical insurance benefits evaluation and coordination; management of the CYSHCN Pool of Funds; information and referral to CYSHCN resources; family-to-family support; and training and consultation with community providers on CYSHCN issues.<\/p>\n<p><strong>Eligibility<\/strong><\/p>\n<ul>\n<li>Birth to 21 years of age<\/li>\n<li>Virginia Resident<\/li>\n<\/ul>\n<p><strong>Physical Eligibility<\/strong><br \/>\n<em>Children with special health care needs who have disorders that:<\/em><\/p>\n<ul>\n<li>Have a physical basis<\/li>\n<li>Have lasted, or are expected to last, at least 12 months; and produce one or more of the following:\n<ul>\n<li>Need for health care and ancillary services over and above the usual for the child's age, or for special ongoing treatments, interventions, or accommodation at home or school<\/li>\n<li>Limitation in function, activities, or social role in comparison with healthy age peers in the general areas of physical, cognitive, emotional, and social growth and development;<\/li>\n<li>Dependency on one of the following to compensate for, or minimize limitation of, function, activities, or social role: medications, special diet, medical technology, assistive devices or personal assistance.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>Financial Eligibility (Required only for those seeking assistance from the CSHCN Pool of Funds)<\/p>\n<p>The funds assist with payment of certain services such as medications, diagnostic testing, therapies, durable medical equipment, and hospitalizations.<br \/>\n<em>A limited amount of money is available to assist those who are:<\/em><\/p>\n<ul>\n<li>Uninsured or underinsured<\/li>\n<li>Whose families have gross family income at or below 300% of the Federal Poverty Level<\/li>\n<\/ul>\n<h3>Preauthorization is required for the use of CSHCN Pool of Funds<\/h3>\n<p><strong>Emergency Preparedness Resources for People with Special Health Care Needs or Disabilities<\/strong><\/p>\n<ul class=\"list\">\n<li><a href=\"https:\/\/www.ready.gov\/individuals-access-functional-needs\">www.ready.gov\/individuals-access-functional-needs<\/a><\/li>\n<li><a href=\"http:\/\/www.redcross.org\/prepare\/location\/home-family\/disabilities\">www.redcross.org\/prepare\/location\/home-family\/disabilities<\/a><\/li>\n<\/ul>\n<\/div>\n<\/div><\/div><\/div><\/div><div id=\"pg-4-1\"  class=\"panel-grid panel-no-style\" ><div id=\"pgc-4-1-0\"  class=\"panel-grid-cell\" ><div id=\"panel-4-1-0-0\" class=\"so-panel widget widget_sow-video panel-first-child\" data-index=\"1\" ><div\n\t\t\t\n\t\t\tclass=\"so-widget-sow-video so-widget-sow-video-default-c08170c47cfb-4\"\n\t\t\t\n\t\t><h3 class=\"widget-title\">Medical Transition for Youth with Special Needs or Disabilities:<\/h3>\n<div class=\"sow-video-wrapper\n\">\n\t\t\t<iframe loading=\"lazy\" title=\"What is Health Care Transition?
